To Use the Features you must have a subscription. After you have created your subscription, refresh the YouTube Home page if you were on it already so extension refreshes with your account.

Sometimes it will "change" a video it has already changed so some clicks might feel like they haven't done anything, that is because it is set to change a random video to recreate the feel of your own Thumbnail being displayed on YouTube!
