Adds a convenient document outline with links to each section in an Apply Form by Avela

This extension updates the document navigation map for the sections of an Avela Form in the Apply application. It is helpful when forms are very long to be able to quickly jump between sections. The navigation map stays visible on the page as you navigate up and down the form.
With v1.5, the Verifications section is now "sticky" as you scroll through the form.
With v1.6, a "copy form ID" button has been added.
Finally, it adds some visual helpers, highlighting required input fields and making more prominent the links for uploaded documents and their verification buttons.
