Extension creates TFS items from service now items

This item opens the new work item page in the specified TFS project with some information auto-populated based on the service now incident you're currently on.
This item is extremely narrow in terms of what configurations of service now it supports and what version of TFS it supports.
You are certainly welcome to see if this works for you, but it's mostly designed for a single narrow use case.
I'll post the source code soon so you can tailor this to work for you if it only works partially.
